EVALUATION INSTRUCTIONS: After payment, you will receive a confirmation email with instructions to email Dr. Ascani (dr.ascani@evolvemindwellness.com) with your name and date of birth and he will get you scheduled for the evaluation. A $275 evaluation fee will be collected directly on his website when you book.
If for whatever reason you are not cleared medically to participate, you will receive a full refund of $850 + $50 medication and medical supplies fee but will forfeit the evaluation fee of $225.
If you cancel after August 1, you will receive a 50% refund.
