EVALUATION INSTRUCTIONS: After payment, you will receive a confirmation email with instructions on how to schedule with Dr. Ascani. You will need to send him your name and date of birth and he will get you scheduled for the evaluation. A $275 evaluation fee will be collected directly on his website when you book.
If for whatever reason you are not cleared medically to participate, you will receive a full refund of $650 + $50 medication and medical supplies fee but will forfeit the evaluation fee of $225.
If you cancel after August 1, you will receive a 50% refund.
